What is Advance Care Planning?

Advance Care Planning involves two main things: appointing a Substitute Decision-Maker and
creating an Advance Care Directive.
A Substitute Decision-Maker (also known as a medical treatment decision-maker) can
consent or refuse medical treatments if you are unable to make your own decisions.
